To make the most out of your sessions with the vet physio centre, we must ask that all relevant paperwork be completes before your first appointment. This has to be completed at least once unless there is a gap of 6 months or more in-between treatments.

By Checking, I understand that vet physio centre sessions do not replace veterinary advice or expertise

By checking, I confirm that my vet is in agreement for my dog to attend Vet Physio Centre sessions

By checking, I confirm that should my dog become ill/injured between gaining vet permission and Vet Physio Centre session, I will inform both parties
